By enrolling as a DNA affiliate technician with Urban DNA Diagnostics, you acknowledge and agree that you will adhere to all applicable laws and regulations governing DNA sample collection and handling.

Urban DNA Diagnostics is not liable for any misuse or mishandling of collected samples or data. Affiliates are responsible for obtaining proper consent and ensuring the accuracy of collected information.

Urban DNA Diagnostics reserves the right to terminate the affiliation if any violations or misconduct are detected.

PROFESSIONAL AFFILIATE AGREEMENT

This Professional Affiliate Agreement ("Agreement") is made and entered into on , by and between Urban DNA Diagnostics Mobile DNA ("Company"), with its principal office located in West Haven, Connecticut, and [Affiliate's Name] ("Affiliate"), with an address at [Affiliate's Address].

1. AFFILIATE PROGRAM DESCRIPTION
The Company offers a Professional Affiliate Program allowing affiliates to market and upsell Urban DNA Diagnostics’s testing services for profit. The Affiliate will receive specified commissions for each service sold.

2. SERVICES AND PRICING
The Affiliate is authorized to sell the following Urban DNA Diagnostics services at the specified retail prices:

• Buccal Swab Collections: $300-$500

• Non-Invasive DNA Prenatal Test: $1,695

• Infidelity Test: $225

• Grandparents Test: $400

• Unusual Items Test: $450

• Post-Mortem Test: $950

The Affiliate may upsell these services at a price above the retail price, retaining the difference as profit.

3. COLLECTION SERVICES AND PROCESS
Upon booking, Urban DNA Diagnostics will dispatch a certified DNA specimen collector to the client’s location to complete the required services. The Company will issue a collection receipt via email for every completed collection, providing confirmation and details of the service provided.

4. COMPANY FEE AND PAYMENT TERMS
The Affiliate is responsible for collecting all payments from clients. The Company’s fee, as outlined in the retail prices above, is due prior to the collection service. Payment must be made immediately upon booking by utilizing the specified payment links provided by the Company.

5. AFFILIATE RESPONSIBILITIES
The Affiliate agrees to:

• Market and promote Urban DNA Diagnostics’s services with professionalism and integrity.

• Ensure that all client payments are processed using the specified payment links.

• Provide clients with accurate information regarding the services and prices.

• Immediately notify the Company of any issues or disputes related to the services or payments.

6. NON-COMPETE AGREEMENT
The Affiliate agrees not to promote, market, or sell DNA testing services from any other company or entity during the term of this Agreement. This non-compete clause ensures the Affiliate’s efforts are focused solely on Urban DNA Diagnostics's offerings.

7. NON-DISCLOSURE AGREEMENT (NDA)
The Affiliate agrees to maintain the confidentiality of all information related to the Company’s operations, pricing, clients, and any other proprietary information. The Affiliate shall not disclose, share, or use any such information for any purpose other than performing the services under this Agreement.

8. TERMINATION
This Agreement may be terminated by either party at any time, with or without cause, by providing written notice to the other party. Upon termination, the Affiliate will be entitled to any commissions earned up to the date of termination, provided that the Company’s fee has been fully paid for each transaction.

9. INDEPENDENT CONTRACTOR STATUS
The Affiliate acknowledges that they are an independent contractor and not an employee of the Company. As such, the Affiliate is responsible for their own taxes, insurance, and any other legal obligations.

10. GOVERNING LAW
This Agreement shall be governed by and construed in accordance with the laws of the State of Connecticut.

11. ENTIRE AGREEMENT
This Agreement constitutes the entire understanding between the parties and supersedes all prior discussions, agreements, or understandings of any kind.
